2. Weigh at least 110 lbs (50 kg)
3. Be in generally good health
4. Bring a valid photo I.D., proof of address, and Social Security number (Social Security Card, W2, or pay stub showing full name and SS#) or Border Crossing Card.
Center Information
Biomat USA Inc. is located in historic Birmingham, AL, also known as the "Magic City". The center is located on the north side of the city and is easily accessible from freeways and near the airport. We look forward to your visit!
